The Order Form is the binding document executed during the subscription signup process.

What is the Order Form?

The Order Form is a subscriber-specific document generated during the Focal Dataworks subscription signup process. It is the document you electronically accept, and it incorporates the following documents by reference:

What Does the Order Form Include?

Each Order Form is unique to the subscribing organization and contains:

  • Subscriber information — Organization name, authorized signer name, title, and email
  • Subscription terms — Monthly subscription rate, billing cycle, and service tier
  • Promotional terms — If applicable, any promotional pricing or discount details
  • Purchase order number — Optional PO# for organizations that require it
  • Incorporated documents — List of documents incorporated by reference with their versions
  • Effective date — The date the Order Form is executed

How is the Order Form Executed?

The Order Form is presented during the subscription signup process as part of the Subscription Agreement Packet. You review the full MSA, Schedule A, Privacy Policy, and User Terms before electronically accepting the Order Form via our ESIGN Act-compliant clickwrap process.

Upon acceptance, a complete record is created including:

  • Full text snapshot of the MSA and Order Form at the time of signing
  • SHA-256 document integrity hash
  • Multi-timestamp audit trail (page load, consent, document review, acceptance)
  • Signer identity and IP address

A signed PDF copy of your Order Form is available for download from your account dashboard after execution.
